PT-141 (Bremelanotide) (PT-141)

Standard manufacturer protocol: 8–12 clicks (400–600 mcg) subcutaneously several hours before intimacy (1 click = 50 mcg). Set the pen programmer to "8", "10", or "12". Effect develops within 1–6 hours.

Alternative regimen for extended window: 8 clicks (400 mcg) in the evening + 5 clicks (250 mcg) in the morning — effect lasts up to 30 hours.

Subcutaneous injection in the abdomen or front of the thigh. After injection, wait 10 seconds before withdrawing the needle. Rotate injection sites.

PT-141 (Bremelanotide) is a synthetic peptide, an analog of α-MSH (melanocyte-stimulating hormone). FDA-approved under the trade name Vyleesi for treating reduced sexual desire in women (HSDD, premenopausal). Unlike PDE5 inhibitors (Viagra, Cialis) that work on the vascular system, PT-141 acts directly on the central nervous system — activating melanocortin receptors MC3R/MC4R in the hypothalamus and triggering natural neurobiological pathways of sexual arousal.

In studies, PT-141 has shown potential in enhancing libido in both men and women, improving erection in men in phase 2 studies (intranasal form, including sildenafil non-responders — Safarinejad & Hosseini, J Urol 2008; development of that form was stopped in 2008 over blood-pressure concerns, and there is no FDA approval for men), enhancing sexual sensations and orgasm quality, and supporting confidence and self-esteem. The effect appears within several hours and, per manufacturer (Pen Peptide) data, lasts up to 24–30 hours depending on the regimen. Research is ongoing into bremelanotide combined with tirzepatide (GLP-1/GIP) for obesity (phase 2, NCT06565611) and a co-formulation with a PDE5 inhibitor for ED (phase 3 announced by Palatin).

PT-141 (Bremelanotide) is a synthetic cyclic heptapeptide, an agonist of melanocortin receptors MC1R, MC3R, and MC4R. Key mechanism for enhancing libido: in preclinical studies (rats), activation of presynaptic MC4R in the medial preoptic area (mPOA) of the hypothalamus increases dopamine release — the excitatory neurotransmitter of sexual motivation; blocking MC4 or D1 receptors in the mPOA abolishes this effect (Pfaus et al., J Sex Med 2007; review: CNS Spectrums 2021). Fundamental difference from PDE5 inhibitors (Viagra/Cialis): PT-141 acts through the CNS (central mechanism), not through the vascular system — in phase 2 studies (intranasal form) it showed an effect even in sildenafil non-responders — 33.5% vs 8.5% placebo (Safarinejad & Hosseini, J Urol 2008); there is no FDA approval for men. FDA approved Bremelanotide under the brand Vyleesi in 2019 for treating HSDD (hypoactive sexual desire disorder) in women — RECONNECT clinical trials (n=1267 randomized): statistically significant increase in sexual desire and reduction in distress (Kingsberg et al., Obstet Gynecol 2019).
