Melanotan II (Меланотан 2)

Melanotan II is NOT FDA-approved and has one of the worst safety profiles among cosmetic peptides. Common side effects: nausea (50–70%), flushing, spontaneous erections, decreased appetite, darkening and enlargement of moles. Serious risks: possible melanoma acceleration in people with dysplastic nevi — case reports exist. Absolutely contraindicated with >20 moles, family history of skin cancer, pregnancy. Before course MANDATORY — dermatologist examination with photodocumentation of all moles (mole mapping) and repeat monitoring after course. PT-141 (Bremelanotide) — safer MT-II "derivative" — is FDA-approved (2019) only for libido (without pigmentation). If goal is only sexual health — choose PT-141.

After complete loading (5-7 days) + 2-3 weeks maintenance, tan lasts 3-6 months. For maintenance — 0.25 mg once/week during summer season. Without maintenance, pigmentation slowly fades over 4-8 weeks.

Nausea from MT-II results from MC4R activation in hypothalamus and nausea neurons. Strategies: (1) start with 0.1 mg and gradually increase; (2) inject in evening (so nausea occurs at night); (3) take ondansetron 4 mg 30 min before injection; (4) don't eat 1 hour before and 2 hours after. Nausea decreases by day 5-7 of course.

Different goals: MT-II — primarily for tanning (MC1R dominance), PT-141 — primarily for libido (MC4R selectivity). MT-II has "bonus" ↑libido but stronger side effects (nausea, pigmentation). PT-141 developed from MT-II to have libido effect without mole pigmentation. For libido — PT-141 safer, for tanning — MT-II only option.

Categorically not indicated: >30 moles on body, dysplastic nevi, family history of melanoma, active oncology or remission <10 years, pregnancy, lactation, children, uncontrolled hypertension, cardiovascular diseases, epilepsy. Relatively not indicated: depression, anxiety disorders, endometriosis (due to ↑libido and hormonal action).

1 Pen Peptide 10mg pen (200 clicks × 50 mcg) completely covers standard 4-week loading cycle + maintenance phase 6–8 weeks — total 2–3 months. Calculation: loading phase uses approximately 24 injections × 8 clicks = 192 clicks. So for full annual tanning care, 1–2 pens per year (1–2 cycles) are sufficient. Exact amount determined by DOZA consultant after skin type and goal assessment.

Nausea with MT-II occurs in 50–70% of users through MC4R activation in brain's chemoreceptor trigger zone. Minimization strategies: 1) EVENING injection — so side effect occurs during sleep. 2) Food intake 30–60 min before injection. 3) Start with 4 clicks (200 mcg) first 3 days, then raise to 8. 4) Antihistamines (Cetirizine, Loratadine) 30 min before injection — partially reduces. 5) Ginger tea after injection. 6) For very severe nausea — Ondansetron (Zofran) 4 mg 30 min before injection (by doctor's prescription). In most cases, nausea weakens after 5–7 days — body adapts.

Melanotan II and PT-141 (Bremelanotide) are "brothers": PT-141 derived from MT-II in 2000s. Key difference: MT-II gives tanning + libido + appetite reduction (via MC1R+MC4R); PT-141 — only libido without pigmentation (selective MC4R agonist). If goal is cosmetic (tanning) — MT-II; if only sexual health (erectile dysfunction, female HSDD) — PT-141. PT-141 is safer: FDA-approved (2019), no mole risks, no melanoma problem. MT-II — only for healthy people with fair skin without atypical nevi and without family skin cancer. In 2026, trend toward abandoning MT-II in favor of PT-141 + sunless tanning lotions (DHA) for skin.
