Genuine Mounjaro or counterfeit: how to tell
We put a genuine Mounjaro by Eli Lilly and a counterfeit side by side. See the difference in the photos.
Short answer
Counterfeit quality has improved: packaging, holograms, and even the DataMatrix code can be copied. However, print quality, cartridge membrane, and cap mould marks always reveal a copy. Below are four signs we documented on real samples.
No visual inspection replaces verifying the batch origin. These signs filter out crude fakes, but the best protection is purchasing through a verified supplier with a documented chain of custody.
Why it matters now
Tirzepatide popularity has grown exponentially. Along with demand, the counterfeit market has expanded: OLX, Telegram channels, and unlicensed 'pharmacies' offer Mounjaro 30–50% cheaper. Some of this product consists of visually decent copies with unknown contents.
We collected several samples marketed as 'genuine UK' and performed a side-by-side comparison with Mounjaro obtained through a licensed distributor.
Sign 1: batch number mismatch
Genuine — the batch on the KwikPen and the outer box always match. On counterfeits the pen batch often differs from the box: counterfeiters print boxes separately from what goes inside.
Sign 2: packaging print quality
Genuine Eli Lilly packaging is offset-printed — dots are invisible without a magnifier, gradients are smooth. Counterfeits are digitally printed: under magnification you see a coarse halftone dot, duller colours, and blurred text edges.
Sign 3: cartridge membrane
The hardest part to counterfeit is the cartridge itself. A genuine Mounjaro cartridge has a distinctive ribbed grid texture on the rubber membrane (septum). On counterfeits the membrane is smooth and glossy. You can see it by removing the grey pen cap.
Sign 4: cap mould marks
The inside of a genuine KwikPen cap has even mould marks from Eli Lilly's press tool. Counterfeits show rougher casting marks, burrs, or missing internal mould identification.
What no longer works as verification
Visual inspection. Counterfeit quality has improved — you can't always tell the box apart by touch or sight.
GS1 DataMatrix. The code on a counterfeit can be valid — copied from a genuine batch.
Eli Lilly QR code. Scan.lilly.com doesn't cover UK packs; even for US serials the response doesn't guarantee the pen contents.
Price. Counterfeiters set a 'market' price to avoid suspicion.

Don't use it
Don't inject. Even if it 'looks fine' — if there's even one mismatch, stop.
Keep everything
Don't discard the box, pen, or thermobox. Photograph batches, DataMatrix, membrane.

Questions about authenticity
Does a visual check guarantee authenticity?
No. Visual inspection filters out crude fakes, but a quality copy can look identical. The guarantee comes from verified batch provenance through a licensed distributor.
What if the batch on the box and pen differs?
Don't use it. Batches must always match. A mismatch is the most reliable sign of a counterfeit or repackaged product.
How do I check a GS1 DataMatrix?
Scan it with any 2D code scanner. The code must contain GTIN, batch number, expiry date, and serial number. These must match the text on the box exactly.
Why isn't the counterfeit cheaper than the original?
Counterfeiters set a 'market' price to avoid suspicion. A low price isn't the only indicator, and a high price is no guarantee of authenticity.
Scan.lilly.com can't find my serial — is it a fake?
Not necessarily. Scan.lilly.com verifies US-version serials. For UK packs (common in Ukraine) the service won't return a result — and that doesn't mean it's counterfeit.
What guarantee does DOZA offer?
