What is Mounjaro®?
— is a medication based on tirzepatide by Eli Lilly, available in pen injectors
Approved for weight loss — not just for diabetes
- • FDA (USA), November 2023 — approved tirzepatide under the name Zepbound® for treating obesity (BMI ≥30 or ≥27 + comorbidities)
- • EMA (European Union), March 2024 — approved Mounjaro® also for weight management in adults without diabetes
So if you don't have diabetes — that's fine. Mounjaro is officially prescribed for weight loss.
Mounjaro® KwikPen® is a single-use, prefilled pen injector for multiple doses. The pen contains 4 fixed doses, with one dose injected weekly. Administer one subcutaneous injection once a week.
Important note. Needles are used for Mounjaro® injections. Recommendations for needle selection and prescriptions can be obtained from a specialist.
How does Mounjaro® work?
Mounjaro® mimics the action of both gut hormones (GIP and GLP-1) in the body to regulate blood sugar levels.
Improves insulin release when blood sugar levels are high
Reduces food intake and hunger
Helps the body better eliminate excess sugar from the blood
Food stays in the stomach longer (this effect diminishes over time)
Affects glucose metabolism in the liver
The body responds better to its own insulin

Mounjaro Instructions: How to Use Mounjaro KwikPen®?
All steps must be performed with every injection!

Remove the cap and inspect the pen
Remove the protective cap from the pen and check the pen and its label.

Take a new pen needle
- →Wipe the red inner seal with an alcohol swab
- →Always use a new needle for each injection — to avoid infections and needle clogging
- →Remove the protective paper from the outer needle cap

Attach the needle to the pen
- →Press the needle in its protective cap vertically onto the pen and twist until firmly attached

Remove the needle caps
- →Remove the outer protective cap of the needle and save it — it will be reused
- →Remove the inner protective cap of the needle and dispose of it
Remove air from the Mounjaro® KwikPen® pen

Slowly turn the dose knob until you hear 2 clicks and an extended line appears in the dose window (air purge position)
Hold the pen with the needle pointing up
Gently tap the cartridge holder so that air bubbles collect at the tip
Release a small amount of medicine into the air by pressing the dose knob all the way in, slowly count to 5. The symbol ⑩ should appear in the dose window
Air is removed, if a small amount of medicine comes out of the needle tip. If not — repeat no more than 2 times. If still no medicine — replace the needle and repeat (no more than 1 time).

Choose the injection site
- →Self-injection should preferably be into the thigh or abdomen (at least 5 centimeters from the navel)
- →Alternatively, another person can inject into the back of the upper arm
- →Rotate injection sites weekly. You can use the same body area, but choose a different spot within that area

Turn the dose knob
Turn the dose knob all the way until the symbol ① appears in the dose window. The ① symbol indicates a full dose.

Inject Mounjaro®
- →Insert the needle into the skin
- →Inject the medicine by pressing the dose knob all the way in. Then slowly count to 5 while holding the knob. The symbol ⑧ should appear in the dose window
- →Remove the needle from the skin. A drop of medicine on the needle tip is normal and does not affect the dose
- →If bleeding occurs — gently press the injection site. Do not rub
Check: If the ⑧ symbol is in the window — a full dose was delivered. If not — reinsert the needle and complete. Do not change dose settings.

Completion and needle disposal
- →Carefully put the outer protective needle cap back on
- →Unscrew the needle with the outer cap on and dispose of the needle in a puncture-resistant container
- →Do not store the pen with the needle attached — to prevent medicine leakage, needle clogging and air entry
- →Put the pen cap back on. Do not store the pen without the cap
Tips for possible side effects
Any medication can cause side effects, and Mounjaro® is no exception. The most common are nausea and diarrhea. They mainly occur during dose adjustment and are usually mild to moderate. These effects most commonly occur at the beginning and typically resolve over time in most patients.
🤢 Tips for better tolerability
If you experience nausea after injection, contact your specialist. In the meantime, these tips may help:
Eat small portions
Avoid fatty foods
Drink water and ensure your body stays well hydrated
Other possible side effects
Mounjaro® has virtually no risk of low blood sugar (hypoglycemia) when used as monotherapy.
However, hypoglycemia may occur when using Mounjaro® with medications containing sulfonylurea and/or insulin. When using these, the dose may need to be reduced while taking Mounjaro®.
