Саксенда (Saxenda) ліраглутид — ін'єкційна ручка поряд з Мунджаро KwikPen, порівняння GLP-1 препаратів для схуднення

When the FDA approved Saxenda in 2014, it was a true breakthrough. The first GLP-1 agonist for weight loss. A medication that finally helped people with obesity not just lose weight, but change their relationship with food.

But since then, Ozempic, Wegovy, and Mounjaro have emerged. And now the question isn't whether Saxenda works. The question is whether it's worth starting with in 2026.

What Is Saxenda and How Does It Work

Saxenda (liraglutide) — woman researching GLP-1 weight loss medication information on smartphone
Understanding the mechanism of action — the first step to making an informed weight loss program choice

Saxenda is the brand name for liraglutide, a synthetic analog of human GLP-1 hormone. This hormone is naturally produced in the gut after eating and signals the brain about satiety.

The problem is that natural GLP-1 breaks down in the body within 1.5–2 minutes. Too fast to have a therapeutic effect. Liraglutide is modified so that its half-life is 13 hours [24]. This allows for one injection per day.

The liraglutide molecule is 97% identical to human GLP-1 [15]. It binds to GLP-1 receptors in the hypothalamus — the brain region that controls appetite and food intake. The result? You feel full faster and think about food less between meals.

ℹ️Simply explained

Imagine appetite as a loud radio constantly reminding you about food. Saxenda doesn't turn off the radio completely, but significantly turns down the volume. You still hear hunger signals, but they no longer dominate your thoughts.

It's important to understand: Saxenda affects appetite, not fat metabolism. The medication doesn't magically "burn" calories. It helps you eat less, and this caloric deficit leads to weight loss.

Saxenda Dosing: Titration Schedule from 0.6 to 3.0 mg

Saxenda is a medication with mandatory titration. You cannot start with the maximum dose immediately. The body needs time to adapt to the new GLP-1 level.

The titration schedule spans 5 weeks [16]:

WeekDaily DosePurpose
10.6 mgStarting dose — nausea prevention
21.2 mgGastrointestinal tract adaptation
31.8 mgBeginning of noticeable appetite reduction
42.4 mgPreparation for maximum dose
5+3.0 mgTherapeutic dose (maintenance)

Why this approach? Gradual dose increases minimize the risk of gastrointestinal side effects [12]. Nausea — the most common complaint at the start — occurs significantly less frequently with proper titration.

⚠️Important

If you cannot tolerate a dose increase at any stage, the FDA recommends staying at the current dose for an additional week before the next increase **[23]**. Don't rush the titration.

For adults, the therapeutic dose is specifically 3.0 mg. Lower doses are intended exclusively for titration and don't provide sufficient effect for weight loss [16]. If a patient cannot tolerate 3.0 mg even after slow titration, the doctor may recommend a different medication.

For adolescents 12–17 years old, the starting dose is the same — 0.6 mg daily. But titration may take up to 8 weeks, and the maximum tolerable dose may be lower — 2.4 mg if 3.0 mg causes unacceptable side effects [22].

How to Properly Inject Saxenda — Step-by-Step Instructions

How to inject Saxenda — proper subcutaneous injection technique of liraglutide into abdomen
Subcutaneous injection into the abdomen — the most common Saxenda injection site

Saxenda is administered subcutaneously — under the skin, not into muscle or vein. It's a simple procedure that patients perform independently at home.

Injection sites:

  • Abdomen (most common site)
  • Front of thigh
  • Upper arm
💡Injection site rotation

Change the injection site with each shot within your chosen area **[19]**. This reduces the risk of lipodystrophy — skin lumps (cutaneous amyloidosis) that can impair medication absorption.

Step-by-step instructions [40]:

  1. Preparing the needle. Remove the cap from the pen and attach a new needle. Before first use of a new pen, always check the medication flow.
  2. Selecting the dose. Turn the dose selector to the required value. Don't count clicks — use the numbers on the indicator.
  3. Administering the medication. Insert the needle under the skin at a 90° angle. Press the dosing button and hold until the counter shows "0". Then slowly count to six before withdrawing the needle — this ensures the full dose is delivered.
  4. Disposing of the needle. Never recap a used needle. Dispose of it in a special sharps container for medical waste.

Timing: Saxenda can be injected at any time of day, regardless of meals [20]. However, it's preferable to choose one time and stick to it daily for stable medication levels in the blood.

Critical: Never share your pen with others, even if you've changed the needle [26]. This can lead to transmission of infections.

Saxenda Effectiveness: How Much Can You Really Lose

Let's look at real numbers from clinical trials. Without exaggeration and without understatement.

63.2%
of patients lost ≥5% weight
Джерело [2]
11.7%
average weight loss among therapy responders
Джерело [2]

In the key SCALE Obesity and Pre-diabetes study, 63.2% of patients on Saxenda lost at least 5% of body weight over 56 weeks compared to 27.1% on placebo [2]. This is clinically significant weight loss that improves health outcomes.

But there's a nuance. These 63.2% are so-called "responders." The remaining patients either didn't respond to therapy or responded weakly.

Among those who responded to Saxenda treatment, average weight loss was 11.7% [2]. Among non-responders — only 1.7%. The difference is enormous.

🔬SCALE data

The SCALE study included over 5,000 participants across three Phase III trials **[2]**. Beyond weight loss, patients showed improvements in fasting glucose, blood pressure, and other cardiovascular risk factors.

Regarding weight maintenance: in the SCALE Maintenance study, 81.4% of participants receiving liraglutide were able to maintain their lost weight, and approximately half continued losing weight during the 56 weeks [6].

But it's important to understand the context: these numbers were obtained in clinical trial conditions with regular monitoring and dietary support. Real-life results may differ.

Saxenda Side Effects and Who Should Not Use It

Saxenda side effects — nausea and abdominal discomfort at the start of liraglutide treatment
Mild nausea at the start — a temporary effect that decreases with proper titration

Saxenda has a predictable side effect profile characteristic of all GLP-1 agonists. Gastrointestinal complaints are first and foremost.

Side effect frequency in clinical trials [7], [11]:

Side EffectSaxendaPlacebo
Nausea39.3%13.8%
Diarrhea20.9%9.9%
Constipation19.4%8.5%
Vomiting15.7%3.9%
Abdominal pain5.4%3.1%

The good news: most of these effects are temporary. Nausea typically decreases as the body adapts to the medication [7]. Slow titration significantly reduces the risk.

In studies, about 68% of patients on Saxenda reported gastrointestinal disturbances compared to 39% on placebo [9]. Severe GI side effects were recorded in 4.8% of Saxenda patients versus 1.4% on placebo [9].

⚠️Pancreatitis — rare but serious risk

In clinical trials, pancreatitis was recorded in a small number of patients. If severe abdominal pain that doesn't go away appears, you should immediately stop taking the medication and see a doctor **[9]**.

Who should not take Saxenda:

  • People with personal or family history of medullary thyroid carcinoma
  • Patients with Multiple Endocrine Neoplasia type 2 (MEN 2)
  • Pregnant women and those who are breastfeeding
  • People already taking other GLP-1 agonists (for example, Ozempic)
  • Patients with severe gastrointestinal diseases

Saxenda Reviews: What Patients Say

Clinical data is one thing. But what do real people experience?

In the SCALE study, beyond objective measures, patient quality of life was assessed. Those who lost at least 5% of body weight reported significant improvements in well-being, energy levels, and body satisfaction [2].

A typical patient experience on Saxenda looks something like this:

First 2 weeks: Mild nausea, especially after eating. Appetite begins to decrease, but the effect isn't stable yet.

Weeks 3–4: Nausea decreases. You start noticing that portions have naturally become smaller. Thoughts about snacking between meals become less frequent.

Weeks 5–8 (on maximum dose): Stable appetite reduction. Many patients describe this as "normal" relationships with food — for the first time in years.

Months 3–6: Noticeable weight loss. Improved health markers (if there were issues with blood pressure, glucose).

But there's another side. Some patients report fatigue, especially at the beginning. Daily injections become a psychological barrier for some. And there are those who simply don't respond to therapy — their appetite remains unchanged.

Saxenda, Ozempic, or Mounjaro: Effectiveness Comparison

Saxenda vs Ozempic vs Mounjaro comparison — three GLP-1 weight loss medications
Three generations of GLP-1 agonists: Saxenda, Ozempic, and Mounjaro

Now — the most important question. How does Saxenda compare to newer medications?

GLP-1 Agonists for Weight Loss Comparison

Evidence baseSCALE (2015)STEP (2021)SURMOUNT (2022)
Discontinuation due to side effects14.8%4.3%~5%

The numbers speak for themselves.

In the head-to-head STEP 8 study, semaglutide 2.4 mg (the active ingredient in Wegovy/Ozempic) showed weight loss of 15.8% over 68 weeks compared to 6.4% on liraglutide 3.0 mg [1], [3]. Almost 2.5 times the difference.

2.5x
more effective semaglutide vs liraglutide
Джерело [1]

And that's not all. In the STEP 8 study, only 4.3% of patients on semaglutide discontinued treatment due to side effects compared to 14.8% on liraglutide [1]. So the newer medication is not only more effective but also better tolerated.

Mounjaro goes even further. Tirzepatide activates two receptors simultaneously — GLP-1 and GIP — which provides an even more powerful effect. In the SURMOUNT-1 study, weight loss on the maximum dose was 22.5% over 72 weeks.

ℹ️Why is the difference so large?

It's about molecular structure. Semaglutide has a longer half-life and higher affinity for the GLP-1 receptor. Tirzepatide adds GIP activation, which enhances the metabolic effect **[3]**.

Why More Patients Are Switching to More Powerful GLP-1s

If Saxenda works, why change anything? There are several compelling reasons.

1. Convenience of use

Daily injections mean 365 shots per year. Ozempic and Mounjaro mean 52 shots. For many people, this is a critical difference in compliance (adherence to treatment regimen).

2. Higher effectiveness

With the same effort, you get 2–3 times better results. For someone who has been struggling with excess weight for years, this can mean the difference between "losing 8 kg in a year" and "losing 20 kg in a year."

3. Better tolerability

Paradoxically, newer medications are often better tolerated. In the STEP 8 study, the rate of treatment discontinuation due to side effects was three times lower on semaglutide [1].

4. Availability

Historically, Saxenda was the only option. Now Mounjaro, Biopatid, and other products are available in Ukraine. The choice has expanded.

Not sure which GLP-1 is right for you?

It's worth noting: Novo Nordisk, the manufacturer of Saxenda, has essentially discontinued production of the branded medication in some countries [3]. The company has shifted focus to Wegovy — their new-generation semaglutide product.

Who Saxenda May Still Suit

Despite the emergence of more powerful alternatives, Saxenda hasn't disappeared from the arsenal. There are situations where it remains an appropriate choice.

Patients with intolerance to semaglutide or tirzepatide. If you've tried newer medications and couldn't tolerate them, liraglutide may be an alternative with a different side effect profile.

Those who prefer a daily regimen. Yes, for most people, less frequent injections are an advantage. But some patients psychologically better accept a daily ritual: "Every day I'm doing something for my health."

Adolescents 12–17 years old. Saxenda has FDA approval for this age group [15]. In a study of adolescents with obesity, the medication showed a BMI reduction of 4.29% over 56 weeks [15].

Patients with limited budgets. In some countries, generic versions of liraglutide have appeared (for example, Nevolat in the United Kingdom [3]), which may be cheaper than branded medications.

⚕️Medical disclaimer

Choosing a weight loss medication is always an individual decision that should be made together with a doctor. Effectiveness and tolerability differ from person to person. What works for one patient may not suit another.
