When the FDA approved Saxenda in 2014, it was a true breakthrough. The first GLP-1 agonist for weight loss. A medication that finally helped people with obesity not just lose weight, but change their relationship with food.

But since then, Ozempic, Wegovy, and Mounjaro have emerged. And now the question isn't whether Saxenda works. The question is whether it's worth starting with in 2026.

What Is Saxenda and How Does It Work

Saxenda is the brand name for liraglutide, a synthetic analog of human GLP-1 hormone. This hormone is naturally produced in the gut after eating and signals the brain about satiety.

For adolescents 12–17 years old, the starting dose is the same — 0.6 mg daily. But titration may take up to 8 weeks, and the maximum tolerable dose may be lower — 2.4 mg if 3.0 mg causes unacceptable side effects [22].

How to Properly Inject Saxenda — Step-by-Step Instructions

Saxenda is administered subcutaneously — under the skin, not into muscle or vein. It's a simple procedure that patients perform independently at home.

1. Preparing the needle. Remove the cap from the pen and attach a new needle. Before first use of a new pen, always check the medication flow.
2. Selecting the dose. Turn the dose selector to the required value. Don't count clicks — use the numbers on the indicator.
3. Administering the medication. Insert the needle under the skin at a 90° angle. Press the dosing button and hold until the counter shows "0". Then slowly count to six before withdrawing the needle — this ensures the full dose is delivered.
4. Disposing of the needle. Never recap a used needle. Dispose of it in a special sharps container for medical waste.

Timing: Saxenda can be injected at any time of day, regardless of meals [20]. However, it's preferable to choose one time and stick to it daily for stable medication levels in the blood.

Critical: Never share your pen with others, even if you've changed the needle [26]. This can lead to transmission of infections.

First 2 weeks: Mild nausea, especially after eating. Appetite begins to decrease, but the effect isn't stable yet.

In the head-to-head STEP 8 study, semaglutide 2.4 mg (the active ingredient in Wegovy/Ozempic) showed weight loss of 15.8% over 68 weeks compared to 6.4% on liraglutide 3.0 mg [1], [3]. Almost 2.5 times the difference.

And that's not all. In the STEP 8 study, only 4.3% of patients on semaglutide discontinued treatment due to side effects compared to 14.8% on liraglutide [1]. So the newer medication is not only more effective but also better tolerated.

Mounjaro goes even further. Tirzepatide activates two receptors simultaneously — GLP-1 and GIP — which provides an even more powerful effect. In the SURMOUNT-1 study, weight loss on the maximum dose was 22.5% over 72 weeks.
